Above is a release, which allows you to indicate your preference on whether or not Itty Bitty Kickers has permission to use photographs and/or video of your child. Itty Bitty Kickers uses photographs and videos of our sessions with you and your children for a variety of projects and media which include promotional materials and social media networks such as Facebook and Instagram. Because we are sensitive to the safety and privacy of your family, at no time will the names of our children accompany their photo or video image without your consent.

As the parent or legal guardian of the child registered, I give permission for my child to participate in Itty Bitty Kickers. I also agree to indemnity Itty Bitty Kickers and staff from any injury that may occur to child and/or adult while participating in the event at Itty Bitty Kickers. I have read and understood the foregoing and acknowledge my consent to the terms of this Waiver & Assumption of Risk by checking the box above.
